My nine year old went to the dentist recently and he missed a bad cavity. How is that possible?

It's only 2 months later and my daughter just noticed a 2mm groove in a molar! Wouldn't the dentist have sunk his pick into a soft tooth if he had checked thoroughly? Is a baby tooth of no concern? The guy had the nerve to charge $160 too!
